Level One Imaging Solutions, Inc. ("Level One Imaging Solutions," "Level One," "we," "us," or "our") respects your privacy. This Privacy Policy explains how we collect, use, disclose, retain, and protect personal information when you visit leveloneimaging.com, use an online service that links to this policy, request information or service, purchase products or services, communicate with us, or opt in to receive text messages (collectively, the "Services").

This policy does not apply to a third-party website or service that has its own privacy policy, even when that website or service is linked from our website. Please review the privacy practices of those third parties before providing information to them.

1. Personal Information We Collect

The personal information we collect depends on how you interact with us and may include:

  • Service, equipment, and transaction information: service requests, equipment manufacturer, model, serial number or asset identifier, equipment location, service history, scheduling information, parts or order details, shipping information, quotes, purchase information, invoices, and payment status.
  • Communications: information included in website forms, emails, telephone calls, text messages, support requests, surveys, or other communications with us.
  • SMS information: mobile telephone number, date and method of opt-in, messaging preferences, consent records, opt-out requests, and message delivery or response information.
  • Website and device information: Internet Protocol (IP) address, browser type, device type, operating system, referring and exit pages, pages viewed, dates and times of visits, approximate location derived from an IP address, and similar technical information.
  • Account information: username, account contact information, authentication information, and account activity if an online account is made available.
  • Other information you choose to provide: any other information submitted in connection with an inquiry, request, application, contract, or business relationship.

Payment-card and similar payment information may be collected directly by a payment processor. When a third-party processor handles a payment, its privacy policy and security practices govern its processing of payment information.

4. How We Disclose Personal Information

We may disclose personal information to the following categories of recipients for legitimate operational purposes:

We do not sell personal information for money. We do not share personal information for cross-context behavioral advertising. We have not knowingly sold or shared the personal information of individuals under 16 years of age.

The disclosures described above do not permit a recipient to use your mobile telephone number, SMS opt-in information, or messaging consent for its own marketing or promotional purposes. Our specific SMS privacy commitments are described below.

5. Cookies, Analytics, and Embedded Content

Our website may use cookies, server logs, pixels, local storage, and similar technologies to operate the website, remember settings, understand website usage, maintain security, and improve performance. We may also use embedded content or features provided by third parties, such as maps, videos, or website analytics. Because there is not a uniform industry standard for browser "Do Not Track" signals, our website may not respond to those signals. Where required by applicable law, we will recognize legally valid universal opt-out preference signals, such as Global Privacy Control, for processing to which the signal applies.

6. SMS and Mobile Messaging Privacy

When you opt in to receive text messages from Level One Imaging Solutions, we may collect your mobile telephone number, messaging preferences, and record of consent. We use this information to send requested service-related communications, including service scheduling, technician status, service-report updates, parts and order information, and shipping notifications.

We do not share, sell, rent, or provide your mobile telephone number, SMS opt-in information, or messaging consent data to third parties or affiliates for marketing or promotional purposes.

All other information-sharing provisions in this Privacy Policy exclude mobile telephone numbers, SMS opt-in information, and messaging consent data when the sharing is for marketing or promotional purposes. SMS consent is specific to Level One Imaging Solutions and will not be transferred to another business for that business's marketing or promotional messages.

We may use contracted communications service providers solely to operate and deliver our messaging program. Those providers may process mobile information only as necessary to provide services to Level One Imaging Solutions and may not use it for their own marketing or promotional purposes.

You may opt in through a signed Level One Imaging Solutions SMS Text Messaging Consent Form or another affirmative opt-in method that we clearly present and document. Consent to receive text messages is not a condition of purchasing goods or services. We do not use purchased lists to enroll recipients in our SMS program.

Message frequency varies. Message and data rates may apply. Reply STOP to any message to opt out. After you opt out, we may send one confirmation message and then will stop sending messages to that number unless you provide new consent. Our standard SMS program is intended for business and service coordination. Do not send patient names, patient records, protected health information, payment-card information, passwords, or other highly sensitive information by text message.

7. Marketing Communications and Your Choices

You may unsubscribe from a marketing email by using the unsubscribe link in the email or contacting us. You may opt out of SMS messages as described in Section 6. Even after you opt out of marketing, we may continue to send non-promotional communications that are necessary to provide requested services, manage an existing transaction or account, respond to you, or comply with law.

8. Privacy Rights

Depending on where you live and subject to applicable law, you may have the right to request that we:

  • Confirm whether we process your personal information and provide access to it.
  • Correct inaccurate personal information.
  • Delete certain personal information.
  • Provide a portable copy of certain personal information.
  • Identify the categories of personal information collected, the sources, the purposes for collection or use, and the categories of recipients.
  • Opt out of a sale, targeted advertising, or sharing for cross-context behavioral advertising, if we engage in that processing.
  • Limit certain uses or disclosures of sensitive personal information, where applicable.
  • Withdraw consent when processing is based on consent.
  • Appeal a decision concerning a privacy request, where applicable.

Please include enough information for us to identify the records involved and understand your request. We may need to verify your identity and authority before completing a request. An authorized agent may submit a request when permitted by law, but we may request proof of authorization and identity verification.

California residents may also request information under California's "Shine the Light" law regarding certain disclosures of personal information to third parties for their own direct-marketing purposes. Level One Imaging Solutions does not disclose mobile telephone numbers or SMS opt-in and consent information to third parties for their own direct marketing.

9. Data Retention

We retain personal information only for as long as reasonably necessary for the purposes described in this policy, including providing services, maintaining service and quality records, completing transactions, managing warranties or contracts, documenting consent, resolving disputes, enforcing agreements, protecting security, and complying with legal, tax, accounting, insurance, and regulatory requirements. Retention periods vary according to the type of information and the applicable purpose.

We use reasonable administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, loss, misuse, alteration, or disclosure. No website, electronic transmission, or storage system can be guaranteed to be completely secure. 11. Patient and Health Information

Our public website, general contact forms, ordinary email accounts, and standard SMS program are intended for business communications about diagnostic imaging equipment and related services. They are not intended for patient care, medical advice, or the transmission of patient records or protected health information. Please do not submit patient-identifying information through these channels. If sensitive information must be exchanged for an authorized business purpose, contact us first so that an appropriate method can be arranged.

12. Children's Privacy

Our Services are intended for businesses and adults and are not directed to children under 13. We do not knowingly collect personal information from children under 13 through the Services. If you believe a child has provided personal information to us, please contact us so we can review and, when appropriate, delete it.
